This policy describes how Signature Medicine handles information collected through signaturemedicinemd.com. Protected health information (PHI) collected during clinical care is governed separately by our HIPAA Notice of Privacy Practices, which we provide at enrollment.

This policy covers information collected when you visit signaturemedicinemd.com or interact with us through web forms, email, or the patient portal link. It does not cover health information collected during clinical visits — that information is governed by the federal Health Insurance Portability and Accountability Act (HIPAA) and our Notice of Privacy Practices, available at the office and provided at enrollment.
2. What we collect
We collect only what we need to operate the website and respond to inquiries:
- Information you provide: name, email, phone, message, and any details you choose to include when you submit a contact or consultation-request form.
- Automatically collected: standard server logs (IP address, browser type, referring URL, pages visited, timestamps) and limited analytics events described below.
- Cookies and similar technologies: we use a small number of cookies for essential site functionality (e.g., remembering accessibility preferences) and for analytics. You can disable cookies in your browser; some features may not work without them.
We do not ask for, and you should not submit through web forms, sensitive clinical detail (diagnoses, medications, conditions). Web forms are not a secure channel for that information.
3. Analytics and measurement
We use Google Analytics 4 and Ahrefs Web Analytics to understand how visitors use the site so we can improve it. These services may set cookies and record events such as page views, button clicks, and consultation-form submissions. We do not link analytics data to any individual’s identity.
You can opt out of Google Analytics by installing the Google Analytics opt-out browser add-on or by using your browser’s privacy / Do Not Track settings.
4. How we use the information
We use information collected through the site to:
- Respond to your inquiry and, if applicable, schedule a consultation.

6. Data retention
Form submissions are retained for as long as needed to respond to your inquiry and meet our record-keeping obligations. Analytics data is retained per the analytics provider’s default settings. You may request deletion of your inquiry data at any time by emailing info@signaturemedicinemd.com.
7. Security
We use industry-standard safeguards — HTTPS encryption in transit, restricted-access systems for form submissions, and reputable cloud infrastructure (Vercel) — to protect information collected through the site. No system is perfectly secure, and we cannot guarantee absolute security. If you discover a security issue, please email info@signaturemedicinemd.com immediately.
8. Children’s privacy
The site is not directed to children under 13, and we do not knowingly collect personal information from children under 13. If you believe a child has provided us information through the site, contact us and we will delete it.
9. Your rights
Depending on your state of residence, you may have rights to access, correct, delete, or request a copy of personal information we hold about you, and to opt out of certain processing. To exercise these rights, email info@signaturemedicinemd.com. We will respond within the time frame required by applicable law.
10. HIPAA notice (clinical care)
Protected health information collected during clinical care is governed by HIPAA and our Notice of Privacy Practices, which describes how we may use and disclose PHI, your rights regarding your PHI, and how to file a complaint. The Notice is provided at enrollment and available at the office.
11. Changes to this policy
We may update this policy from time to time. The “Last updated” date reflects the most recent revision. Material changes will be reflected on this page; continued use of the site after a change takes effect constitutes acceptance of the updated policy.
